• Temporary checks are not accepted. Only checks with pre-printed information can be processed.
• Write your order number on the memo line of the check.
• Write your telephone number on the check.
• Write your driver's license number on the check, and indicate the state the license was issued by. If you do not provide a driver's license number, we cannot process your check order. You will have to pay by money order instead.
• If your check is preprinted with a PO Box address, write your valid street address on the check. Also, remember we cannot ship to a PO Box.
• Include a copy of your order with the check
• Print this form, and fill in all information.  Any missing information will cause your order to be cancelled.
• Include this completed form with your check.
